Static Control Services Endstat 2000 Blower Pulse DC Ionizer.
The only thing I can tell you is that when plugged in the fan works. There is a green and red flashing light on the right side of the box behind the "screen"  The heat button does not stay on when pushed so I am unsure if it needs replaced.
